Summary

Abstract

The study focuses on peripheral nerve injuries involving the radial, axillary, and femoral nerves, defined as "acute compressive neuropathies" resulting from shibari practice.

To explore the nature and extent of these nerve issues, the authors conducted an exploratory study with 10 practitioners, documenting 16 injuries affecting the radial, axillary, or femoral nerves.

These injuries appeared suddenly, typically after full-body suspensions.

The radial nerve was most affected, with 90% of participants experiencing damage.

One illustrative case involved repeated acute compression during suspension: a 29-year-old woman suspended for 25 minutes developed radial “drop” (loss of wrist and finger extension) and numbness in the left hand.

Nerve conduction testing revealed a 77.3% conduction block in the upper arm. Function improved within three months and fully recovered by five. Seventeen months later, bilateral radial compression recurred after an 8–10 minute suspension; she recovered within a week, completely by four. A third episode three years later resolved fully within two weeks.

Given that the radial nerve is the most frequently affected, the study underscores the importance of understanding its anatomical course—particularly its location posterior to the distal deltoid tuberosity—to help prevent injury.

Such anatomical awareness is invaluable for shibari practitioners seeking to reduce nerve injury risk.

On the Study Itself

Although exploratory and based on a limited cohort, this research marks an important reference point for both shibari practitioners and clinicians.

It represents the first instance in medical literature connecting compressive neuropathies directly to shibari (Japanese rope bondage).

Its physiological discussions on demyelination and axonal impact align closely with established models of compressive nerve pathology.

The findings highlight the critical overlap between anatomy and shibari technique and emphasize understanding the mechanics underpinning safety.

However, the authors’ limited familiarity with shibari concepts and terminology occasionally results in interpretive bias—especially framing injuries exclusively around full suspensions when similar stressors may occur in grounded ties.

Despite these gaps, the paper’s contributions are significant for grounding shibari-related injury patterns in documented medical analysis.

What Stands Out from This Study

A key concern is the evident disparity between technique quality and anatomical awareness shown by participants. Most exhibited unsafe practice habits and poor mechanical control.

Unfortunately, social media evidence suggests these issues are widespread rather than exceptional within the shibari community.

Shibari is a biomechanical discipline, not improvisation. It requires scientific understanding, skill development, and disciplined technique, not intuition or mimicry.

Proficiency takes months of consistent study and years of refinement. Attempting suspensions without sufficient grounding in anatomical mechanics is both reckless and potentially abusive.

Competent instruction should prioritize mechanical understanding, nerve safety, and responsible pedagogy—not rushed, superficial workshops that compromise student welfare.

Only through rigorous training, accountability, and adherence to biomechanical safety principles can the practice of shibari evolve responsibly while preserving its human and artistic dimensions.
